Univita Termination with All Statewide Medicaid Managed Care Plans
The Statewide Medicaid Managed Care (SMMC) plans will no longer contract with Univita for home health, durable medical equipment (DME) or intravenous therapy for their Medicaid members.
Providers should continue providing services to enrollees during the transition period for any services that were previously authorized or prescheduled, regardless of whether the provider is participating in the plan’s network. The Agency expects SMMC plans to honor any existing prior authorizations of ongoing covered services and will ensure SMMC plans pay for authorized services during this transition period.
Effective immediately, Univita will no longer process new prior authorization requests. If you need to request a prior authorization or are interested in contracting with a SMMC plan, please refer to the listing below.

United Healthcare and Univita Terminating Their Durable Medical Equipment/Home Health/Home Infusion Subcontract effective 8/1/15 |
Effective August 1, 2015, United Healthcare (United) will no longer be contracted with Univita for durable medical equipment, home health care, and home infusion services for their Managed Medical Assistance (MMA) program in Florida. Due to this network change, as of July 1, 2015, Univita stopped accepting new referrals however they will manage all current authorizations for existing services through 07/31/2015.
United has contracted directly with these provider types to service their enrollees. For a full listing of their most current local and national network providers, please visit United’s website at www.uhccommunityplan.com or call 877-842-3210.
Continuity of care provisions will be in effect for this change. If a United enrollee is in active care they may keep seeing that provider for up to 60 days after August 1, 2015. Authorizations/Notifications may be required for some services. To request authorizations please contact United at 866-604-3267or fax requests to 866-607-5975.
