January 1, 2020, the Lonsurf Copay Assistance Program will transition to a new copay administrator

Starting January 1,2020, the Lonsurf Copay Assistance Program will transition to a new copay administrator.   It will require some administrative actions on the part of whomever processes the scripts, both refills of existing scripts and new scripts.    

• Currently enrolled, commercially insured eligible patients will move to the new program:
  • Member ID # will remain the same
  • New Group # 1003001
  • New BIN # 600428
  • NEW PCN# 79011
• New patients may be enrolled into the program by:
  • Calling 844-824-4648 to determine eligibility and enroll over the phone   (This number has not changed)
  • Going to www.TaihoOncologyCopay.com and enrolling online.    (If you or any of your staff go to the old portal address, you will receive an error message referring you to the new portal)
